Our Services

The case management department promotes a collaborative process that assesses, plans, implements, coordinates, monitors and evaluates options and services to meet a patient's health care needs. Our customers include patients, families, physicians, other health care team members, community health and human service providers, and payers. Practioners in the department include a director, medical social workers, RN case managers RN care managers, and administrative support.

Services provided:

• Discharge planning intervention and implementation of continuum of care plans.
• Patient and family conferences and supportive counseling.
• Utilization management.
